- Marital Status (Married/Living Together/Civil Partners/Divorced)
- Number of dependent children, and their ages
- Does anyone other than your partner and/or dependent children live with you?
(If ‘yes’, then note down details, including their relationship to you, if applicable) - Are you working? If yes, then how many hours per week?
- Is your Partner working? If yes, then how many hours per week?
- Total household income (Include maintenance payments and any other income) £
Is income net or gross ?
Frequency (Weekly/Fortnightly/ Four weekly/Calendar monthly) - Savings or other capital £
- Do you own your own home?
If yes, do you have a mortgage, and if so how much is it? - Are you currently claiming any benefits? (Yes/No/Not now but was recently)
If yes, list them with amount and frequency of payment - Note brief details of your enquiry
